Refer a Patient to Hospice

Family Physican
Medical Director
Family Member
Clergy
Nursing Home or Assisted Living Center Staff
Friends
Guardian

Upon receiving a referral, Hospice of Central Ohio will contact the patient and family to arrange for an admission consultation. Admission consultations are conducted at the location of the patient and family's choosing. Consultations are conducted 24 hours a day, 7 days a week.

During the consultation the patient and family will be provided with a complete outline of the hospice program, services offered and have the opportunity to ask all of the questions they may have.
